Ockenden report: 500+ mothers and babies harmed at Nottingham NHS Trust

Donna Ockenden's independent review found that more than 500 mothers or babies died or were harmed at Nottingham University Hospitals NHS Trust. The inquiry says a 'toxic' bullying culture and a 'cruel', dismissive attitude to women contributed to avoidable deaths and injuries, and leaders were aware of serious problems from at least 2010 but failed to take action. Health Secretary James Murray vowed the government and NHS leaders would "deliver lasting change" after publication, and the report follows similar failings uncovered at Morecambe Bay, East Kent, and Shrewsbury and Telford. The scale and persistence of these failures demand urgent reforms, independent investigations, and accountability across NHS maternity services to prevent further harm.
